COMMUNITY SERVICE OPPORTUNITY ! ! !

My name is Anel Sirois I am the clinical coordinator for Mt Ascutney Physician Practice and OHC clinics.  I would like to extend an invitation to  students who need community service hours to volunteer to be a part of our yearly Flu Clinics.  Volunteers would be a great  help to guide our patients to the correct exam rooms for their flu injections.

We need two student volunteers per clinic. Weekday clinics are 2 hours long and Saturday clinics are 3 hours long.

The dates for the clinics are     Saturday 9/24/16 from 9am to 12pm
                                                     Thursday 10/6/16 from 5pm to 7pm
                                                      Saturday 10/15/16 from 9am to 12pm
                                                      Wednesday 10/26/16 from 5pm to 7pm
                                                      Saturday 11/12/16 from 9am to 12pm

We ask students report to the clinic 30 minutes prior to the start of clinic and locate any one of the employees working the flu clinics.  I will leave student names with the staff running the clinics on the day they volunteer to know to expect them.

Our practice manager will write a letter stating how many hours each student performed community service for as proof of service.  Enclosed is a confidentiality agreement we ask the students to read through, sign, and bring to the clinic the day they are volunteering.
